Our organization is looking for a full time Social Worker at Miller County Hospital. 

POSITION SUMMARY:  

The duties and responsibilities of the Social Services Worker include providing a medically-related social service program to meet the needs of patients and residents, determining the type of assistance required, assisting in obtaining benefits from agencies and organizations such as Medicaid, Social Security, Veterans benefits and others, completing the necessary forms and involving families whenever possible and advisable, etc. Utilizing community resources and preparing for discharge, in accordance with federal, state and local regulations and the current established policies and procedures of the facility. Will work closely with ACO team and Hospital’s interdisciplinary team caring for inpatients or swing-bed patients.

 Qualifications:

• Social Worker degree preferred- 4 year or Masters level 
• One year working in a nursing home, hospital, health care facility or other social service industry preferred.
• Strong background and knowledge in Medicaid preferred
