POSITION PURPOSE:
Under the direction of Case Management, serves as a coordinator of an interdisciplinary approach to guide patients through the continuum of care with special focus on discharge planning, psychosocial needs assessment, care/resource management, and provider/insurance issues. Collaborates with the physicians, patients, and their family members and others to facilitate the achievement of desired outcomes, address ongoing needs, maximize patient, family and provider satisfaction, optimize systems/resource utilization, facilitate timely discharge, assure appropriate length of stay, reduce unnecessary delays and other adverse variances to the delivery of services.
POSITION QUALIFICATIONS:
Education:
Bachelor’s degree in Social Work or a Human Services Field.
Experience:
Minimum of one to two years of clinical social work experience in a hospital or health care setting which may have included the appropriate supervision or internship.
