Provides crisis intervention and assists families in understanding the implications and complexities of the life situation and its impact on lifestyle. Provide social work services to patients and families to assist them in continuity of care planning and in enhancing their capability of coping with illness and daily functioning. Assist the individual and family with psycho social needs which may compromise adaptation.
Essential Duties & Responsibilities:
Education: Bachelor's degree in social work or related field required. Master's degree in social services preferred.
Experience: 3 year's experience in general social services required.
Knowledge of basic social agencies and resources within the community. Knowledge of medical terms and processes. Knowledge and experience working with Child Protective Services as well at Hospice initiation. Some OB/Peds experience preferred.
