Responsibilities

The Social Worker is responsible for patient advocacy, emergent counseling and education needs for patients and families, and effective discharge planning within Population Health system.

Qualifications

Job Requirements:

• Bachelor or Master Social Work degree

Preferred Job Requirements:

• Previous hospital experience
