Qualifications
· Current unrestricted RN license in the state of Indiana
· Valid driver's license and auto insurance in your name as a driver
· Two years as a Registered Nurse with at least one-year of management experience in a home care, hospice or equivalent environment
· Ability to exercise initiative and independent judgment
· Ability to work with individuals to enlist cooperation of many people to perform/achieve a common goal
· Compassionate communicator with a positive attitude
· Attention to detail is critical, as is being observant and following directions
· Critical thinker and the ability to problem solve clinical needs
Responsibilities
· Assists the Clinical Manager with the supervision of home health services to patients in their place of residence in accordance with physician orders along with state and federal regulations
· Strives to provide the highest quality of care
· Skilled nursing services furnished are under the supervision of the Clinical Manager
· Understands and adheres to established Agency policies and procedures, as well as state and federal regulations
· Ability to supervise, guide and develop nursing skills and performances
· Assists in recruiting, hiring, evaluating and terminating nursing personnel
· Assists in the coordination of interdisciplinary team services, patient care and referrals
· Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, cooperation and job performances
· Assists with the Agency's ongoing Quality Assessment and Performance Improvement (QAPI) program
· Reviews patient medical records for compliance with federal, state, accrediting bodies, local and Agency policies and guidelines
· Assists Clinical Manager in conducting patient care conferences, staff meetings and maintains documentation
· Other duties as assigned by Clinical Manager/Administrator
· Acts as Clinical Manager in the Manager’s absence
· Participate in on-call rotation
