    Description of Work Unit
    Medical Services
    The Medical Clinic at the CSU Health Network provides acute and chronic care to CSU students by appointments and on a walk-in basis for urgent concerns. Primary care physicians, physician assistants, and nurse practitioners care for students, with emphasis on preventive care, education, and continuity. The medical staff are supported by a full range of laboratory, radiology, pharmacy, physical therapy, psychiatry, optometry, and nutrition services on site, behavioral health and integrated care with counseling staff enhances the treatment of students with behavioral health concerns. Minor procedures are performed in the clinic. The Medical Clinic provides services between the hours of 8:00 am – 5:00 pm Monday-Friday, providing attractive work schedules for staff.

    Position Summary
    The Behavioral Health and Care Coordination (BHCC) Manager collaborates with the Medical Directors and the CSU Health Network staff to provide high-quality healthcare and behavioral health interventions to CSU students. The BHCC Manager supervises the Behavioral Health Care Coordinators. The position participates in the visioning, development, and implementation of collaborative care models across the CSU Health Network. The position collaborates with others within the CSU Health Network to implement concepts of universal design to ensure that students with marginalized identities have access to services.

    The Behavioral Health and Care Coordination Manager reports directly to the Director of Medical Clinical Services.

    Required Job Qualifications
    Education/License
    • Master’s degree in Social Work
    • Licensed to practice (Licensed Clinical Social Worker) in the State of Colorado or able to obtain licensure within 30 days of beginning employment
    Experience
    • Minimum of two years of post-master’s degree experience providing clinical mental health services
    • Experience with crisis management, safety/risk assessment, and situation de-escalation
    • Experience caring for individuals with marginalized identities

    Preferred Job Qualifications
    Experience
    • At least one year of supervisory experience
    • Experience providing brief clinical interventions
    • Experience, training, and knowledge in Primary Care Behavioral Health (PCBH) model of integrated care
    • Experience providing collaborative multi-disciplinary healthcare and coordination of care between Medical Services Psychiatry Services, and Counseling Services providers
    • Experience working with a young adult/college age population

    • Provides behavioral health services within the primary care setting, including brief functional assessment via contextual interview, validated behavioral health screening, brief evidence-based interventions, and crisis support for students referred by the Medical Services staff
    • Provides integrated care for psychological and health-related needs, such as insomnia, pain management, substance use, stress, somatization, chronic health conditions, health behavior change, mood and anxiety symptoms, and trauma-related concerns
    • Maintains accurate and timely documentation for all clinical interactions and consultations
    • Provides outreach, liaison, and consultation services consistent with Integrated Behavioral Health Model
    • Collaborates with medical providers and staff to promote patient and population health according to trauma-informed, person-centered, strengths-based, anti-oppressive, and culturally responsive principles of care
    • Participates in case review conferences
    • Provides care coordination, including identification of community and campus resources and appropriate referrals to those resources
    • Manages triage of psychotropic medication requests and provides consultation with respect to medication management, including triage to urgent/crisis appointments
    • Supports facilitation of transfers of care, admissions, and referrals
    • Other duties as assigned

    • Develops onboarding procedures and training for Behavioral Health Care Coordinators
    • Manages schedules to ensure adequate staffing in all assigned clinical areas, including the approval and tracking of time and leave
    • Mitigates daily issues regarding delivery of behavioral health and care coordination services
    • Plans and leads regular staff meetings and staff training in principles and practices of integrated care
    • Establishes and continuously evaluates care and treatments to ensure evidence-based, patient/client-centered care consistent with best practices established in the profession
    • May coordinates with Training Director and Clinical Director in Counseling Services to consider training rotations in integrated behavioral health if requested by counseling trainees and approved by the Medical Director
    • May provide clinical supervision of assigned trainees in graduate training rotations in integrated behavioral health in accordance with training policies and procedures, and communicate feedback to the Counseling Services Training Director
    • May provide clinical supervision to Licensed Social Workers (LSW) to help them earn their licensure as Licensed Clinical Social Workers (LCSW)
