Spares Coordination Manager manages the delivery of spare parts and equipment, and scheduling of customer repairs. Negotiates delivery cost for the organization and the end cost to customer. Being a Spares Coordination Manager verifies terms and conditions of contracts are satisfactorily met. Ensures projects are completed on time and within budget. Additionally, Spares Coordination Manager acts as advisor to spares coordination team regarding projects, tasks, and operations. Requires a bachelor's degree. Typically reports to a unit/department head. The Spares Coordination Manager manages a departmental sub-function within a broader departmental function. Creates functional strategies and specific objectives for the sub-function and develops budgets/policies/procedures to support the functional infrastructure. Deep knowledge of the managed sub-function and solid knowledge of the overall departmental function. To be a Spares Coordination Manager typically requires 5+ years of managerial experience.     is looking for a qualified Integrated Care Coordination Supervisor tor join our team in downtown Tacoma WA.

    Function
    :

    Provide leadership and support to the integrated care coordination unit and carry a .5 FTE caseload as a Integrated Care Coordination Supervisor. This program provides support for CLR clients who meet criteria for care coordination by presenting with high-risk physical health indicators based on screening or who do not have access to a primary care physician.


    Responsibilities:

    • Provide daily support to Healthcare Coordinators related to staff training, locating resources; case assignment; etc...
    • Care coordination responsibilities include assessment healthcare service needs, care planning/goal setting with clients, and monitoring clients with acute and chronic conditions through treatment pathways.
    • Work under the supervision of the assigned Manager/Director and track and report agreed upon data metrics outcomes
    • Assist the Manager/Director with hiring and on-boarding new staff
    • Identifies and mitigates barriers to accessing health care and social services.
    • Coordinates and communicates regarding the client’s post-discharge inpatient status with all involved health care providers including, but not limited to primary care, mental health, specialty care, and pharmacy.
    • Provides transition follow up from different levels of care which also support and empower the client to build confidence in four key areas: medication self-management, use of the client’s primary care health record (if applicable and available), primary care and specialist follow-up, and knowledge of red flags of their condition and how to respond.
    • Provides referrals and advocacy for clients and their caregivers to community-based services and support.
    • Works collaboratively with multi-disciplinary teams involving nurses, therapists, case managers and peers.
    • Creates and maintains complete and concise client files in compliance with CLR’s documentation policy.
    • Attends required meetings and trainings.


    Qualifications
    :

    • Bachelor’s degree in social or behavioral sciences required plus two years relevant experience or three years of experience in behavioral or physical healthcare in lieu of a degree.
    • Experience in the provision of case management or care coordination services in a behavioral health or physical health setting.
    • Experience that demonstrates the ability to work with a diverse population of all ages and their families, specifically individuals who have a serious mental illness and who may be unhoused.
    • Experience meeting and working with people in their homes and other medical and community settings is desired.
    • Ability to obtain a Washington State behavioral health provider credential.
    • Valid Washington State driver’s license; proof of insurance. Ability to use one’s own car to visit consumers in the community when no company car is available.
