Speech and Language Pathologist - Home Care assesses and treats speech and language disorders and impairments in a home care setting. Conducts evaluations of an individual's speech and language skills using applicable diagnostic materials and tools. Being a Speech and Language Pathologist - Home Care develops treatment plans that utilize appropriate intervention approaches, strategies, and materials. Establishes treatment priorities that include advising, educating, and counseling. Additionally, Speech and Language Pathologist - Home Care completes required documentation to record assessment and progress. Collaborates with physicians, family members, and other clinical providers to provide effective therapies. Ensures that treatment that is provided aligns with patient needs and is within the guidelines set by the payer. May direct support staff in assisting with treatment. Requires a master's degree in speech pathology. Typically requires Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP). Requires State License based on Clinical Specialty. Position Summary
Direct, supervise, evaluate and provide speech therapy services to patients in the home as prescribed by the attending physician.
Qualifications
· Baccalaureate degree and Masters degree in speech pathology
· Must have "Certificate of Clinical Competency"
· Licensed as required by the New York State Education Department
Job Description
· Evaluate the patient’s speech, language and voice, and swallowing abilities according to referral and perform periodic reevaluation.
· Develop and implement rehabilitative services for speech, language and voice, and swallowing disorders.
· Select and administer diagnostic tests and provide appropriate therapeutic techniques and materials.
· Record the type of treatment and patient’s reaction to it on the clinical/progress notes.
· Maintain adequate records on all of the patients, including summary reports.
· Instruct and educate family members and patient regarding treatment plan and therapy carry-over.
· Attend case conferences.
· Participate in the development of and periodic revision of the physician’s plan of care.
· Other duties as assigned.
