Speech and Language Pathologist - Home Care assesses and treats speech and language disorders and impairments in a home care setting. Conducts evaluations of an individual's speech and language skills using applicable diagnostic materials and tools. Being a Speech and Language Pathologist - Home Care develops treatment plans that utilize appropriate intervention approaches, strategies, and materials. Establishes treatment priorities that include advising, educating, and counseling. Additionally, Speech and Language Pathologist - Home Care completes required documentation to record assessment and progress. Collaborates with physicians, family members, and other clinical providers to provide effective therapies. Ensures that treatment that is provided aligns with patient needs and is within the guidelines set by the payer. May direct support staff in assisting with treatment. Requires a master's degree in speech pathology. Typically requires Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP). Requires State License based on Clinical Specialty. Typically reports to a manager. Speech and Language Pathologist - Home Care's years of experience requirement may be unspecified. ST SUMMARY
Evaluates and treats residents and patients with temporary or long-term speech, hearing, swallowing, and cognitive disabilities in compliance with state and federal guidelines.
ST ESSENTIAL DUTIES AND RESPONSIBILITIES
Under a physician’s direction, provides direct patient / resident treatment based on established plan of care.
Screens and evaluates Resident and develops appropriate care plan
Plans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and / or swallowing or cognitive problems.
Administers, scores, and interprets specialized hearing and speech tests.
Instructs residents, families, and caregivers to monitor speech and provide ways to practice new skills.
Completes required forms and documents in accordance with company policy and state and/or federal regulations.
Maintains patient records in an accurate and timely manner.
Devises special tools and treatments for the specific needs of patients.
ST ENTRY QUALIFICATIONS
Master of Science degree in Speech Language Pathology.
Current license in Speech Therapy as required by state law.
Certification in clinical competency.
One-year clinical experience preferred.
ST SUPERVISORY RESPONSIBILITIES
May supervise Rehab Aides, and others involved in program
