Speech and Language Pathologist - Home Care assesses and treats speech and language disorders and impairments in a home care setting. Conducts evaluations of an individual's speech and language skills using applicable diagnostic materials and tools. Being a Speech and Language Pathologist - Home Care develops treatment plans that utilize appropriate intervention approaches, strategies, and materials. Establishes treatment priorities that include advising, educating, and counseling. Additionally, Speech and Language Pathologist - Home Care completes required documentation to record assessment and progress. Collaborates with physicians, family members, and other clinical providers to provide effective therapies. Ensures that treatment that is provided aligns with patient needs and is within the guidelines set by the payer. May direct support staff in assisting with treatment. Requires a master's degree in speech pathology. Typically requires Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP). Requires State License based on Clinical Specialty. Typically reports to a manager. Speech and Language Pathologist - Home Care's years of experience requirement may be unspecified.   • JOB SUMMARY: 

    A Speech Language Pathologist (SLP) administers speech therapy to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager. Speech therapy services are furnished only by or under the supervision of a qualified speech pathologist or audiologist. 

    QUALIFICATIONS: 

    1. A person who has a master’s or doctoral degree in Speech Language Pathology, and is licensed as a Speech Language Pathologist by the state where they furnish services, or 

    1. A person who has successfully completed 350 clock hours of supervised clinical practicum (or be in the process of completing), at least nine months of supervised full-time SLP experience, and has successfully completed a national examination approved by the Secretary. 

    1. Currently licensed in the state(s) in which practicing. 

    1. Two (2) years’ experience preferred. 

    RESPONSIBILITIES: 

    1. Improves or maximizes the communication of the patient. 

    1. Returns the individual to optimum and productive living within the patient’s capabilities. 

    1. Periodically participates with all other home care personnel in patient care planning. 

    1. Provides full range of Speech Language Pathology Services as ordered by physician. 

    1. Directs and supervises personnel, as assigned. 

    1. Takes initial history and makes initial evaluation. 

    1. Performs all skilled procedures. 

    1. Consults with physicians regarding change of treatment. 

    1. Writes reports to physicians regarding patient’s progress. 

    1. Instructs patients and family members in home programs. 

    1. Periodically presents an inservice to the Agency’s staff members. 

    1. An initial evaluation, including a plan of treatment and goals, must be completed and submitted to the physician for approval. A recertification by a physician every 60 days is necessary if further treatment is to be continued. A progress note is written each visit. A reevaluation is written when the expected duration of treatment is reached. A summary is written upon patient’s discharge.
