Speech and Language Pathologist - Home Care jobs in Richmond, VA

Speech and Language Pathologist - Home Care assesses and treats speech and language disorders and impairments in a home care setting. Conducts evaluations of an individual's speech and language skills using applicable diagnostic materials and tools. Being a Speech and Language Pathologist - Home Care develops treatment plans that utilize appropriate intervention approaches, strategies, and materials. Establishes treatment priorities that include advising, educating, and counseling. Additionally, Speech and Language Pathologist - Home Care completes required documentation to record assessment and progress. Collaborates with physicians, family members, and other clinical providers to provide effective therapies. Ensures that treatment that is provided aligns with patient needs and is within the guidelines set by the payer. May direct support staff in assisting with treatment. Requires a master's degree in speech pathology. Typically requires Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP). Requires State License based on Clinical Specialty. Typically reports to a manager. Speech and Language Pathologist - Home Care's years of experience requirement may be unspecified.     Responsibilities:
    • Conduct appropriate evaluations, assess home environment and identify equipment needs relative to speech/language function
    • Identify functional speech defects and establish a plan of care to improve patient’s function
    • Administer speech therapy program utilizing specialized therapeutic technique and/or equipment and recommend communication devices/aids as indicated
    • Maintain accurate and timely documentation
    • Participate in case conference as required
    • Ensure compliance with all state and federal legal and regulatory requirements
    • Additional responsibilities as assigned
    Qualifications:
    • Master’s degree in Speech Language Pathology with current licensure to practice in state of operation
    • 1 year experience in Speech Language Pathology
    • Home health experience preferred
    • Commitment to clinical and documentation excellence
