Speech and Language Pathologist - Nursing Home assesses and treats speech and language disorders and impairments in a nursing home or long term care setting. Conducts patient evaluation of speech and language skills using applicable diagnostic materials and tools. Being a Speech and Language Pathologist - Nursing Home develops treatment plans that utilize appropriate intervention approaches, strategies, and materials. Establishes treatment priorities that include advising, educating and counseling the patient. Additionally, Speech and Language Pathologist - Nursing Home documents assessment and patient progress. Collaborates with physicians, family members, and other clinical providers. May direct support staff in assisting with treatment. Ensures that treatment aligns with patient needs and is within the parameters set by the payer. Requires a master's degree in speech pathology or equivalent. Requires Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) and may require a state license. Typically reports to a manager or head of a unit/department. Speech and Language Pathologist - Nursing Home's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. Develop treatment plans and programs including short-term and long-term goals in accordance with physician orders. Provide quality care that assists clients to meet goals; complete re-evaluations per requirements.Client Records and Billing - Provide accurate and timely documentation of services rendered utilizing agency EMR. Demonstrate understanding and knowledge of all payer sources and authorization required. Complete and document supervisory visits with the HHA per regulations.Communication - Communicate effectively with clients, families, referral sources, physicians, and the interdisciplinary team. Demonstrate exceptional verbal and written communication skills.Follow-Up Programs - Recommend and assure continuation of appropriate therapeutic/therapist follow-up programs and activities. Develop programs to optimize successful discharge with highest function possible. Make referrals to continuation programs as necessary.Other Duties as Assigned - The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.Education and Experience Required:Must be licensed as a Speech Language Pathologist in the state of Minnesota.Must obtain CPR certification within 6 months from date of hire.Must have a valid driver’s license and use of an insured personal vehicle.Must complete and pass a state required background study.Able to communicate verbally and in writing to the extent required by the position.Able to physically perform the duties required by the position.Required to have the ability to read, write and speak English well enough to communicate with staff, clients and visitors.Obtaining the COVID-19 vaccine or an approved medical or religious exemption is a requirement of employment for this position.Reports To:Regional Clinical Manager
