Speech and Language Pathologist - Nursing Home assesses and treats speech and language disorders and impairments in a nursing home or long term care setting. Conducts patient evaluation of speech and language skills using applicable diagnostic materials and tools. Being a Speech and Language Pathologist - Nursing Home develops treatment plans that utilize appropriate intervention approaches, strategies, and materials. Establishes treatment priorities that include advising, educating and counseling the patient. Additionally, Speech and Language Pathologist - Nursing Home documents assessment and patient progress. Collaborates with physicians, family members, and other clinical providers. May direct support staff in assisting with treatment. Ensures that treatment aligns with patient needs and is within the parameters set by the payer. Requires a master's degree in speech pathology or equivalent. Requires Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) and may require a state license. Typically reports to a manager or head of a unit/department.   • Scope:Responsible for organizing, developing, and directing speech therapy services in accordance with the standards and practice of speech therapy, state and federal regulations, and Knute Nelson’s policies and procedures.Ensure time management that best utilizes Knute Nelson’s resources while meeting client needs.Assure that the highest degree of quality care, customer service, fiscal responsibility, and regulatory compliance is maintained at all times.Key Responsibilities:Direct Client Care - Evaluate clients; obtain and interpret subjective and objective data. Develop treatment plans and programs including short-term and long-term goals in accordance with physician orders. Provide quality care that assists clients to meet goals; complete re-evaluations per requirements.Client Records and Billing - Provide accurate and timely documentation of services rendered utilizing agency EMR. Demonstrate understanding and knowledge of all payer sources and authorization required. Complete and document supervisory visits with the HHA per regulations.Communication - Communicate effectively with clients, families, referral sources, physicians, and the interdisciplinary team. Demonstrate exceptional verbal and written communication skills.Follow-Up Programs - Recommend and assure continuation of appropriate therapeutic/therapist follow-up programs and activities. Develop programs to optimize successful discharge with highest function possible.
