Speech and Language Pathologist - Nursing Home jobs in Schenectady, NY

Speech and Language Pathologist - Nursing Home assesses and treats speech and language disorders and impairments in a nursing home or long term care setting. Conducts patient evaluation of speech and language skills using applicable diagnostic materials and tools. Being a Speech and Language Pathologist - Nursing Home develops treatment plans that utilize appropriate intervention approaches, strategies, and materials. Establishes treatment priorities that include advising, educating and counseling the patient. Additionally, Speech and Language Pathologist - Nursing Home documents assessment and patient progress. Collaborates with physicians, family members, and other clinical providers. May direct support staff in assisting with treatment. Ensures that treatment aligns with patient needs and is within the parameters set by the payer. Requires a master's degree in speech pathology or equivalent. Requires Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) and may require a state license. Typically reports to a manager or head of a unit/department. Speech and Language Pathologist - Nursing Home's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement.     The Grand Rehabilitation and Nursing Center at Guilderland, located in Altamont, NY is currently seeking a Speech Language Pathologist.

    SLP Duties Include:

    • Evaluate Residents and develop a treatment plan that will meet their needs.
    • Choose activities that will help the Residents to carry out their daily works independently.
    • Watch the Residents as they carry out the task given to them.
    • Observe/Assess the progress of the Residents & prepare detailed progress reports.
    • Help in rehabilitating Residents disabled as a result of injury, illness or other problems.
    • Maintains safe and clean working environment.
    • Maintains Resident confidence by monitoring confidential information processing.
    • Maintain documentation of Resident care services by auditing Resident & Dept. records.
    • Maintains a cooperative relationship among healthcare teams & contributes to team effort.

    SLP Requirements Include:

    • Graduate of an accredited Speech Therapy Program.
    • Current New York State License.
    • Experience is a plus.
