Speech and Language Pathologist - Nursing Home assesses and treats speech and language disorders and impairments in a nursing home or long term care setting. Conducts patient evaluation of speech and language skills using applicable diagnostic materials and tools. Being a Speech and Language Pathologist - Nursing Home develops treatment plans that utilize appropriate intervention approaches, strategies, and materials. Establishes treatment priorities that include advising, educating and counseling the patient. Additionally, Speech and Language Pathologist - Nursing Home documents assessment and patient progress. Collaborates with physicians, family members, and other clinical providers. May direct support staff in assisting with treatment. Ensures that treatment aligns with patient needs and is within the parameters set by the payer. Requires a master's degree in speech pathology or equivalent. Requires Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) and may require a state license. Typically reports to a manager or head of a unit/department. Speech and Language Pathologist - Nursing Home's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement.     Foltsbrook Center Nursing & Rehabilitation has immediate per diem positions available for Speech Language Pathologists at our facility on 104 N. Washington St, Herkimer, NY 13350.

    The Speech-Language Pathologist is an essential role in our facility; They must work together with a multidisciplinary team to develop and implement the highest possible levels of care for our residents. A qualified Speech Language Pathology (SLP) candidate will have to comply with the following responsibilities listed below, as well as meet the following requirements listed below.

    Responsibilities

    This position directly oversees all aspects of speech and language pathology; including assessment, treatment planning, implementation and ongoing review, so strong professional competency and experience is essential.
    • Screening short- and long-term residents, evaluating and interpreting data and determining relevant treatment plans.
    • Setting goals in conjunction with interdisciplinary team members. Appropriately document progress and status.
    • Re-evaluating care plans based on new and emergent conditions, as well as resident progress.
    • Responding to individualized requests and making referrals, as needed
    • Training family or caregivers, as needed.
    Requirements
    • Active License and ASHA Certificate of Clinical Competence.
    • Experience in Geriatrics preferred.
    • Must be able to thrive in a team environment.
