Responsibilities:
- Provide direct patient care, including administering medications, performing procedures, and monitoring vital signs
- Conduct patient assessments and contribute information for individualized care plans
- Collaborate with nurse manager to coordinate and manage patient care
- Provide skilled nursing duties, including tracheostomy care/changes, tube feeding, administer medications via g-tube
- Educate clients and their families on various aspects of healthcare, including infection control
- Maintain accurate and up-to-date medical records
- Accompany client to appointments as needed
Experience:
- Valid LPN or RN license
- Proficiency in medical terminology and documentation
- Strong knowledge of patient care principles and practices
- Experience with tracheostomies and g-tubes is a plus
