Job Title: Admissions Registered Nurse (RN) - Nursing Home

Job Overview:

We are seeking a dedicated and compassionate Registered Nurse (RN) to join our team at Charlotte Bay Rehabilitation and Care Center as an Admissions RN. This position plays a pivotal role in ensuring a seamless transition for residents entering our care, providing exceptional nursing care, and facilitating the admission process. The ideal candidate will have strong clinical skills, excellent communication abilities, and a commitment to delivering patient-centered care.

Salary: $80,000

Responsibilities:

Admission Assessment:

• Conduct comprehensive assessments of potential residents to determine their physical, emotional, and psychosocial needs.
• Collaborate with healthcare providers, families, and residents to gather relevant medical history and information to facilitate the admission process.

Care Planning:

• Develop individualized care plans based on the assessment findings, ensuring that they align with the resident's unique needs and preferences.
• Collaborate with the interdisciplinary healthcare team to coordinate and implement the care plan.

Communication:

• Effectively communicate with prospective residents and their families, providing clear and detailed information about Charlotte Bay Rehabilitation and Care Center's services, policies, and procedures.
• Liaise with physicians, hospitals, and other healthcare facilities to facilitate a smooth admission process.

Documentation:

• Maintain accurate and up-to-date documentation of admission assessments, care plans, and any relevant information in the resident's medical records.
• Ensure compliance with regulatory requirements and documentation standards.

Education and Support:

• Educate residents and their families about Charlotte Bay Rehabilitation and Care Center's programs, services, and available resources.
• Provide emotional support and address concerns, fostering a positive experience during the admission process.

Collaboration:

• Collaborate with the admissions team, nursing staff, and other healthcare professionals to streamline and enhance the admission process.
• Participate in regular interdisciplinary team meetings to discuss resident care and admissions-related matters.

Qualifications:

• Current and valid Registered Nurse (RN) license in the state.
• Minimum of 2 years of nursing experience, with a preference for previous experience in admissions or long-term care settings.
• Strong assessment and critical thinking skills.
• Excellent interpersonal and communication skills.
• Knowledge of regulatory requirements related to nursing home admissions.
• Ability to work collaboratively in a team-oriented environment.
