Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Staff Nurse - RN - Home Care Admissions initiates care with service providers. Completes admission, referral, and insurance forms. Utilizes health record management software. Typically requires a bachelor's degree in nursing or equivalent. Requires an RN license. Typically reports to a manager or head of a unit/department. Qualifications: Requires RN with current SC license. Home health and recent med/surg experience preferred; own transportation. ADN candidates must be enrolled in a BSN program within 18 months from date of hire and are required to obtain BSN within four (4) years of date of hire. For current AnMed Health Associate Degree Nurses (ADN) who were hired as RNs prior to January 1, 2013, and already working as an acute care Staff Nurse as of that date, the requirement to obtain BSN within four (4) years will be waived.
