Staff Nurse - RN - Home Care Admissions evaluates patients referred to home care services and determines the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Being a Staff Nurse - RN - Home Care Admissions obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Staff Nurse - RN - Home Care Admissions initiates care with service providers. Completes admission, referral, and insurance forms. Utilizes health record management software. Typically requires a bachelor's degree in nursing or equivalent. Requires an RN license. Typically reports to a manager or head of a unit/department. Staff Nurse - RN - Home Care Admissions's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement.     RESPONSIBILITIES:
    • Completes an initial assessment of patient and family to determine home care needs, including a complete physical assessment and history of current and previous illness(es).
    • Regularly re-evaluates patient nursing needs.
    • Initiates the plan of care and makes necessary revisions as patient status and needs change.
    • Uses health assessment data to determine nursing diagnosis.
    • Develops a care plan involving the patient and the family to establish goals based on nursing diagnosis incorporating therapeutic, preventative and rehabilitative nursing actions.
    • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the Physician. Provides direct patient care as defined in the State Nurse Practice Act.
    • Counsels the patient and family in meeting nursing and related needs.
    • Provides health care instructions to the patient as appropriate per assessment and plan of care.
    • Identifies discharge planning needs as part of the care of plan development and implements prior to discharge of the patient.
    • Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

    QUALIFICATIONS:
    • Associate's or Bachelor’s degree with One (1) to two (2) years of recent acute care experience in an institutional setting. Two (2) years of home health care experience preferred.
    • Must be licensed in the State as a Registered Nurse.
    • Current CPR certification.
    • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
    • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.Prolonged or considerable walking and standing. 
    • Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching.
    • This position requires that the employee is willing and able to meet the physical requirements as necessary.
    • Visual acuity and hearing to perform required skills.
