Staff Nurse - RN - Home Care Admissions evaluates patients referred to home care services and determines the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Being a Staff Nurse - RN - Home Care Admissions obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Staff Nurse - RN - Home Care Admissions initiates care with service providers. Completes admission, referral, and insurance forms. Utilizes health record management software. Typically requires a bachelor's degree in nursing or equivalent. Requires an RN license. Typically reports to a manager or head of a unit/department. Staff Nurse - RN - Home Care Admissions's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. Effective Pay Rate
When you work at SwiftShift, you are paid per your Effective Pay Rate. Effective Pay Rate has 4 major components:
Service Pay - This is the rate paid to you for delivering in-home services to the client.
Client Support Pay - This is the rate paid to you for supporting clients within your team.
Service Continuity Pay - This is the pay that is paid to every employee for every 500 hours of in-home services delivered.
Weekly Team Pay = This is the pay that is paid to every member of the care team who has delivered 30 or more hours of in-home services without any overtime.
On an average the Effective Pay Rates for our caregivers are:
For LPN = $28-$37 an hour
For CNA = $16.5-$22 an hour
For HHA = $14-$19 an hour
For RN = $33-$40 an hour

With a tracheostomy and tube feedings, Garrett requires round-the-clock care. But he doesn't let his challenges define him. Every Friday from 8 pm to 6 am and Saturdays from 6 pm to 6 am, skilled nurses step in to ensure Garrett's comfort and safety. Their dedication allows Garrett to pursue his passions – whether it's tinkering with gadgets or exploring new worlds through his favorite books. With their support, Garrett not only thrives but inspires everyone around him with his resilience and determination to embrace life to the fullest.
