Staff Nurse - RN - Home Care Admissions jobs in Kansas City, MO

Staff Nurse - RN - Home Care Admissions evaluates patients referred to home care services and determines the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Being a Staff Nurse - RN - Home Care Admissions obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Staff Nurse - RN - Home Care Admissions initiates care with service providers. Completes admission, referral, and insurance forms. Utilizes health record management software. Typically requires a bachelor's degree in nursing or equivalent. Requires an RN license. Typically reports to a manager or head of a unit/department. Staff Nurse - RN - Home Care Admissions's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement.     Responsibilities

    Skilled Visits

    Day-to-day patient interaction

    Education on Health Conditions, such as CHF,CODP, Diabetes

    Wound Care

    Medication Management

    Changes dressings, bandages and contraptions as prescribed

    Documentation of patient’s condition and pointing out significant changes, to be reported to the physician

    Working along side other nurses, Physical Therapist, Speech Therapist and/or Occupational Therapist.
