JOB SUMMARY

The Orange VNA is currently hiring for Full Time Home Health RN case manager for the small territory of Orange, Milford, West Haven and Woodbridge. Small case load, about 5 visits a day.

RESPONSIBILITIES

• Provides supervision and oversight to patient care as outlined by State regulation and/or requested by Agency Director.
• Understands the concepts of home health care and the role of the RN as a member of a full discipline health care team
• Provides skilled hands on care to patients in the comfort of their own home
• Performs start of care visits, resumption of care visit and revisits with timely documentation
• Develops individualized Plan of Care for patients
• Provides education to the patient and their family/support person
• On call rotation with other RN team members

Experience/License Requirements

• Minimum of 2 years in Home Care
• Must have a current RN license in CT
• Current drivers license and valid car insurance

Job Type: Full-time

Pay: $80K - $90K a year
