Staff Nurse - RN - Home Care Admissions evaluates patients referred to home care services and determines the types of services to be provided. Interviews the patient and family to determine appropriateness for services. Being a Staff Nurse - RN - Home Care Admissions obtains initial and ongoing referral documentation from physicians, insurers, and providers. Coordinates the development of patient home care plans that specify the frequency and duration of the services provided. Additionally, Staff Nurse - RN - Home Care Admissions initiates care with service providers. Completes admission, referral, and insurance forms. Utilizes health record management software. Typically requires a bachelor's degree in nursing or equivalent. Requires an RN license. Typically reports to a manager or head of a unit/department. Staff Nurse - RN - Home Care Admissions's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. Focus Staff is seeking a traveling Certified Rehabilitation RN, who will provide comprehensive wound assessments and plan of care including goals, implementation, evaluation of outcomes for patients ranging in age from 18 years to geriatric experiencing medically complex conditions, general medical-surgical conditions, with rehabilitation and/or wound care needs.
Contract Assignment - 13 Weeks (Temporary)
Shift: 5x8 Days

- Acts as a resource for the treatment team and clinical staff and assists with staffing as assigned.
- Trains and manages the Wound Care Team Members to include orientation, competency validation and in-service training.
- Organizes and directs the interdisciplinary Wound Care Committee.
- Perform additional duties assigned.
