Staff Nurse - RN - Home Care provides professional nursing care to assigned patients in a home care environment. Records patient histories, performs various diagnostic tests, administers medications, and develops patient home care plans in conjunction with other medical professionals. Being a Staff Nurse - RN - Home Care performs examinations and prescribed procedures, operating and monitoring medical equipment as necessary. Observes patients' progress and records observations in patient medical records. Additionally, Staff Nurse - RN - Home Care promotes patients' independence by teaching patients and families to understand conditions, medications, and self-care skills. Typically requires a bachelor's degree in nursing. Typically reports to a manager or head of a unit/department. Requires a valid state RN license.     RESPONSIBILITIES:
    • Completes an initial assessment of patient and family to determine home care needs, including a complete physical assessment and history of current and previous illness(es).
    • Regularly re-evaluates patient nursing needs.
    • Initiates the plan of care and makes necessary revisions as patient status and needs change.
    • Uses health assessment data to determine nursing diagnosis.
    • Develops a care plan involving the patient and the family to establish goals based on nursing diagnosis incorporating therapeutic, preventative and rehabilitative nursing actions.
    • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the Physician. Provides direct patient care as defined in the State Nurse Practice Act.
    • Counsels the patient and family in meeting nursing and related needs.
    • Provides health care instructions to the patient as appropriate per assessment and plan of care.
    • Identifies discharge planning needs as part of the care of plan development and implements prior to discharge of the patient.
    • Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.

    QUALIFICATIONS:
    • Associate's or Bachelor’s degree with One (1) to two (2) years of recent acute care experience in an institutional setting. Two (2) years of home health care experience preferred.
    • Must be licensed in the State as a Registered Nurse.
    • Current CPR certification.
    • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
    • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.Prolonged or considerable walking and standing. 
    • Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching.
    • This position requires that the employee is willing and able to meet the physical requirements as necessary.
    • Visual acuity and hearing to perform required skills.
