Staff Nurse - RN - Hospice provides professional nursing care to assigned terminally ill patients in a hospice environment. Records patient histories, performs various diagnostic tests, administers medications, and develops patient care plans in conjunction with other medical professionals. Being a Staff Nurse - RN - Hospice performs examinations and prescribed procedures, operating and monitoring medical equipment as necessary. Observes patients' condition and records observations in patient medical records. Additionally, Staff Nurse - RN - Hospice typically requires a bachelor's degree in nursing. Typically reports to a manager or head of a unit/department. Requires a valid state RN license. Staff Nurse - RN - Hospice's years of experience requirement may be unspecified. - Main Office location: Londonderry, NH.
- Service Area: Nashua and Surrounding areas.
- Job Type: Full-time
- Schedule: Monday – Friday
Responsibilities:Patient Care:- Assesses and evaluates patient’s status by:
- Writing and initiating plan of care
- Regularly re-evaluating patient and family/caregiver needs
- Participating in revising the plan of care as necessary
- Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
- Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
- Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
- Provides and maintains a safe environment for the patient.
- Attends interdisciplinary group meetings.
Additional Duties:- Participates in on-call duties.
- Ensures that arrangements for equipment and other necessary items and services are available.
- Supervises ancillary personnel and delegates responsibilities when required.
- Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
- Fulfills the obligation of requested and/or accepted case assignments.
- Actively participates in quality assessment performance improvement teams and activities.
Qualifications:- 2 years' nursing experience, preferably in geriatrics is desirable.

- Degree in Nursing from an accredited institution.
- Proficient with electronic medical records.
- Excellent bedside manner and communication skills.
- Working knowledge of health care laws and regulations.
- Maintain current BLS CPR certification.
- Possess a valid driver's license and own a reliable, insured vehicle.
