Staff Nurse - RN - Long-Term Care provides professional nursing care to assigned patients in a long-term care facility. Records patient histories, performs various diagnostic tests, administers medications, and develops patient long-term care plans in conjunction with other medical professionals. Being a Staff Nurse - RN - Long-Term Care assists physicians during examinations and procedures, operating and monitoring medical equipment as necessary. Observes patients' progress and records observations in patient medical records. Additionally, Staff Nurse - RN - Long-Term Care promotes patients' independence by teaching patients and families to understand conditions, medications, and self-care skills. Typically requires a bachelor's degree in nursing. Typically reports to a manager or head of a unit/department. Requires a valid state RN license. There is an open Registered Nurse (RN) - Long Term Care job inPassaic, NJ. The Full Time Registered Nurse (RN) will be responsible for coordinating care for assisted living and senior housing residents. The Registered Nurse (RN) will conduct routine monthly visits and partner with the NP to review the patient's status and assist in achieving progress towards goals. Depending on acuity the NPwill perform 5-7 assessments per day.Theassessment will be conducted on Medicare Advantage plan members residing in long term care and senior livingfacilities in Passaic, NJ and the surroundingarea. The RNis typically scheduled in onefacility per shift, but may be required to float if necessary.Responsibilities:
- Conduct monthly comprehensive assessments for a Medicare Advantage health plan
- Communicate and collaborate with the NP
- Perform care coordination activities to ensure needs are met
- Performs resident rounds and assists withadvanced care planning
Work Hours:
- Monday and Friday
- 8:00 AM - 5:00 PM
Preferred Experience:
- 1 year of current RNexperience is preferred
- Previous care coordination or long term care experience preferred
Preferred Qualifications:
- Registered Nurse (RN) License in NJ
- Current CPR certification (BLS)
- COVID vaccination required
Education Requirements:
- RN degree from an accredited school of nursing
