The Company:

Accomplished Home Care is a Community Health Accreditation Partner (CHAP) with six office locations in Florida: Ocala, Leesburg, Clearwater, Tampa, Orlando, and Port Charlotte.

The Position:

• Assess the medical, cognitive, emotional, psycho social, safety, developmental status, and psychiatric nursing needs of the patient during the initial nursing evaluation, assesses the appropriateness of care and criteria for acceptance to psychiatric home healthcare, reports the status of all new patients to the Director of Clinical Services and the patient’s M.D./Psychologist.
• Evaluate the patient’s management and compliance with the medical and psychiatric plan of care
• Maintain awareness of indicators in patient’s behavior that call for changes in the treatment modality (crisis, suicidal ideation)
• Initial and ongoing comprehensive assessments of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.
• Participate in case conferences, discuss with the supervisor problems concerning the patients and how they may best be handled
• Adhere to Federal, state and accreditation requirements including Medicare and Medicaid regulations

The Requirements:

• FL RN License
• Current BLS CPR
• One or more years of experience in a community/home health agency setting
• AHCA Level II Background Screening
• Wellsky (Kinnser) software knowledge preferred
