RN Staff Nurse – Home Health

Accra is a 501.c.3 non-profit home health care organization. Service Management Group (SMG) is the administrative company of Accra which provides administrative, payroll and related services to an employee group of more than 4500 individuals including a Fiscal Management Support (FMS) Agency. A FMS agency provides fiscal support to individual and families who qualify for Medical Assistance through the State of MN. Fiscal support includes budgeting, reviewing and approving timecards and expenses, and filing the necessary paperwork with the IRS or State of MN.

General Description:

The RN Staff Nurse-Home Health provides client-centered, quality nursing care to individuals needing support to achieve wellness and independence. Through the nursing process of assessment, planning, implementation, and evaluation the RN Staff Nurse performs duties in accordance with the Minnesota Nurse Practice Standards Act and per physician orders. Routine job duties include: conducting comprehensive assessments, educating client/family caregiver, assuring an effective car plan, coordinating care, supervising home health aides, maintaining timely documentation, and effectively communicating with all parties. The RN Staff Nurse is a professional role model and mentor to staff to assure the delivery of quality services.

Qualifications:

• Current license as a Registered Nurse in the State of Minnesota.
• A minimum of 1-year current experience working in a hospital, skilled nursing facility, or home health setting.
• Effective verbal and written communication, assessment, care planning, teaching, clinical, and disease management skills (e.g., diabetic care).
• Knowledge of nursing care required by older adults and persons with chronic/acute conditions.
• Skilled with INRs, wounds, blood draws, falls prevention, psychosocial issues, IVs.
• Ability to work independently and supervise home health aides.
• Effective organizational, critical thinking, problem solving, communication, and interpersonal skills.
• Familiar with community resources.
• Basic computer skills and ability to use electronic health record system.
• Ability to comply with privacy and HIPAA laws.
• Must pass a background study with the Minnesota Department of Human Services. Have a valid driver’s license, clean driving record and reliable transportation to use for work driving purposes.
