Education/Experience:

• Current NJ RN License
• Valid Driver’s License required
• One-year exp. in home health required

Job Summary: Provide direct care and patient teaching related to the disease process

Job Description:

• Conducts complete physical, social, and environmental assessments on admission to identify patient needs and develop a plan of care, in conjunction with the physician, to achieve optimal patient care outcomes and rehabilitation.
• Observe and assess patient's responses to nursing care, treatments, and patient teaching.
• Provide education to patients and families on medications, health issues, interventions, and health strategies.
• Helps to promote and ensure a safe home environment by working in close collaboration with all members of the health care team.
• Plans for discharge with appropriate resources to ensure safety and well-being.
