Bear Mountain Healthcare is seeking a full time (40hr/day shift) Director of Social

Services for our skilled nursing facility, Bear Mountain of Sudbury in Sudbury, MA.

The Director of Social Services is responsible for initiating and maintaining a

comprehensive social services program on throughout the center. Social Services

is responsible for identifying and providing for each resident’s social, emotional

and psychological needs. The Director will assist in the development of a safe

discharge plan. This position also requires the timely completion of MDS

assessments, 48-hour meetings, initial, quarterly, and annual assessments.

Long-term care experience is preferred; however, we are willing to train the right

candidate!

Requirements:

• LTC/Nursing Home Experience preferred.
• Licensed Required, MSW preferred
• Per MA Department of Public Health regulations, candidates must be fully vaccinated against COVID-19.
