Surgeon - Heart Transplant examines, diagnoses, and surgically treats patients with end-stage cardiac disease. Surgically replaces a diseased or damaged heart with a healthy heart. Being a Surgeon - Heart Transplant reviews patient history and confirms need for surgery. Determines which instruments and method of surgery will be most successful in achieving the desired outcome. Additionally, Surgeon - Heart Transplant may provide medical personnel with direction concerning patient care. May provide in-service training as needed to address new technology in health care treatment. Provides charting in compliance with all laws and regulations. Requires a MD degree from an accredited school. Typically reports to the chief of surgery. Requires a license to practice. General Statement:
The Manager is responsible for planning and coordinating the operations of the Group Home so that the personal needs, preferences and goals of each resident are effectively met.
Supervises:
Direct Support Lead and Support Staff in Group Home
Job Functions:
•Develops and maintains an interactive relationship with each resident that reflects values and principles help by Heart to Heart Home Care.
•May serve as the IHP Coordinator, assisting with and coordinating the development of the Individual Habitation Plan (IHP) and over seeing the implementation of the IHP. Participates in IHP/IEP resident meetings.
•Coordinates service provision within the community in conjunction with the Case Manager in order to meet the individual needs of residents and to achieve each resident's IHP/IEP goals.
•Review all task analyses and documented client training forms.
•Schedules and facilitates all resident meetings.
•Develops and implements strategies for maximizing the inclusion of residents in the life of the Group Home.
•Provides direct care to residents as needed, including the provision of emergency coverage to the Group Home.
•Prepares and submits monthly medication orders for each resident. In the absence of Community Support Staff, when necessary, may administer medication and documents such in compliance with Heart to Heart Home Care's policies and procedures which conform to DDD guidelines. •Schedules all necessary medical appointments and accompanies residents on appointments.
•Orients new residents to basic safety practices in the Group Home and insures that emergency drills are conducted as required.
•Completes all DDD required training and continues to improve skills through training required by Heart to Heart Home Care.
•Cooperates with Heart to Heart Home Care and Division of Developmental Disabilities (DDD) staff during any inspection or investigation.
