The Home Companion Team Leader assists in the day-to-day operations of Greencroft Community Based Services (CBS). Part of the Team Leader’s time will be spent doing direct care, supervisory visits, training schedule, and service plan evaluations.

Essential Functions:

• Provide direct supervision for Home Companion and Caregiver team members involving training and scheduling.
• Create, evaluate, and update service plans for Greencroft at Home (GAH) clients at regularly scheduled intervals, per GAH policy.
• Work with clients, team members, and client’s family members on service concerns, consulting with Assistant Director of CBS, as needed.
• Complete supervised visits for each team member at regularly scheduled intervals, per GAH policy.
• In absence of Assistant Director of CBS, meet with prospective clients to explain services available through Greencroft at Home and enter information into Generations software if services are initiated.
• In absence of Assistant Director of CBS, answer phone, schedule immediate transportation needs.

Position Requirements:

• High School diploma or GED, preferred.
• Must be able to read, write, and follow instructions.
• Valid Driver’s License and proof of car insurance required. No moving violations within the last 3 years.
• Should enjoy working with people, be tactful, and exercise good judgement in dealing with people.

Schedule:

Full Time, on call every other weekend and holiday
