Team Leader - Home Care manages a multidisciplinary team of nurses, therapists, and aides that provide professional services to home care patients. Coordinates clinician schedules and assigns clinicians to patients based on the frequency and expected duration of prescribed treatments and therapies. Being a Team Leader - Home Care monitors utilization, productivity, and tracks patient outcomes. Ensures compliance with agency policies and procedures. Additionally, Team Leader - Home Care typically requires a bachelor's degree. May require an RN license. Typically reports to a director. The Team Leader - Home Care manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. To be a Team Leader - Home Care typically requires 5 years experience in the related area as an individual contributor. 1 - 3 years supervisory experience may be required. QUALIFICATIONS:
Experience with G-Tube Feeding
Trach suctioning, care and canula replacement, oxygen administration, venting experience
Medication Administration
Full knowledge of assessments and treatments
Respiratory treatments
Vest treatments
Sanitation/equipment cleaning
LPN's and RN's are required to be competent in communication skills, observation, assessment and documentation of care and infection control procedures
Ability to handle multiple tasks simultaneously while determining priorities and managing a changing environment
Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community and workplace
Ability to work NOC Shift 9pm - 7am with weekend availability
