We are very excited to announce the opportunity for a Team Coordinator position! This is a key administration position at our Crown Point office.

The Team Coordinator is responsible for the clerical/scheduling duties of staff as well as facilitating the operations of the team.

Essential Duties and Responsibilities:

• Assures that an adequate supply of qualified field staff is available and appropriately assigned.
• Maintains a current availability list, scheduling staff, properly documenting assignments and/or communication, transfers field staff from one case to another.
• Communicates company policies and procedures to staff. Maintains good communication among the staff, clients, referral sources and other teams.
• Maintains an efficient paperwork flow. Makes recommendations to improve efficiency and effectiveness of paper flow.
• Documents all client-related activities in a timely manner.
• Reacts productively to change.
• Responds promptly to patients’ requests, complaints and concerns of patient and professional staff.
• Ensures confidentiality of patient information.
• Attends team meetings and staff meetings to enhance team communication, coordination of services and quality services.
• Participates in performance improvement activities.
• Notifies personnel and community agencies of patient hospitalization or discharge from service when appropriate.
• Completes weekly report sheets and any required paperwork in a timely fashion to ensure agency operating processes and procedures.
• Compiles with state and federal guidelines and agency policies and procedures.
• Participates in on-call responsibilities

Minimum Qualifications

• Bachelor degree preferred
• Healthcare experience preferred
• Must be at least eighteen (18) years or older
• Computer literate with keyboarding skills
