Team Leader - Home Care manages a multidisciplinary team of nurses, therapists, and aides that provide professional services to home care patients. Coordinates clinician schedules and assigns clinicians to patients based on the frequency and expected duration of prescribed treatments and therapies. Being a Team Leader - Home Care monitors utilization, productivity, and tracks patient outcomes. Ensures compliance with agency policies and procedures. Additionally, Team Leader - Home Care typically requires a bachelor's degree. May require an RN license. Typically reports to a director. The Team Leader - Home Care manages subordinate staff in the day-to-day performance of their jobs. True first level manager. Ensures that project/department milestones/goals are met and adhering to approved budgets. Has full authority for personnel actions. To be a Team Leader - Home Care typically requires 5 years experience in the related area as an individual contributor. 1 - 3 years supervisory experience may be required. - Implement the nursing care plan for each patient.
- Initiate preventive and rehabilitative nursing procedures.
- Teach and counsel the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
- Maintain and complete records of observations, treatments and care of patient. Help update the patient plan of care.
- Administer medications and treatments as ordered.
- Demonstrate competent performance of technical skills according to established procedures before performing new skills.
- Active nursing license in Pennsylvania
