We are seeking a compassionate and skilled Home Care Nurse to join FirstLight team. As a Home Care Nurse, you will provide essential medical care and support to patients in the comfort of their own homes. Responsibilities:

• Provide skilled nursing services for patients in their own residence
• Care for individual based on a personalized Plan of Care
• Monitor and track changes in the client’s conditions and needs, and keep the Nurse Manager and any other appropriate personal informed
• Proactively update client’s family and physician on plan of care, health changes, and concerns as appropriate
• Maintain high level quality of care and assures proper maintenance of clinical records following all Joint Commission standards and state, federal regulations
• Participate in agency performance improvement activities and continuously staying informed about changes in the field of nursing and home health care
• Monitor Vitals
• Medication Management
• Wound Care
• Other duties as assigned

Requirements:

• Licensed as a Registered Nurse in the State of Michigan
• Preferred Bachelor’s degree in Nursing
• Certified CPR
• Minimum one (1) year of documented experience required in a home health agency preferred
• Licensed driver with a reliable car and valid automobile insurance. Able to drive a car 2-4 hours per day
• Clean background and criminal record
• Understands regulatory compliance with state and federal regulations
• Adhere to HIPAA and maintain client confidentiality
• Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client. Must be able to lift up to 50 pounds.

Hospital at Home Opportunity

FirstLight Home Care is proud to provide hospital level care at home for patients. This innovative program allows patients who would otherwise require inpatient hospitalization to be treated in the comfort of their own home. Our Registered Nurse Team will provide a range of services from IV therapies, infusions, wound care, medication management, taking vitals and other tasks as order by the patient’s physician.

Responsibilities:

Infusion Support (including peripheral iv starts).

Monitor and track changes in the client’s conditions and needs, and keep the Nurse Manager and any other appropriate personnel informed.

Requirements:

Minimum three (2) years of documented experience required with at least one (1) year in a home health agency, hospital or health facility, preferred.

At least one (1) year of recent IV experience with proficiency in peripheral IV starts

Must have the following vaccinations: COVID-19, Annual Flu Vaccination, Varicella

Up to date TB test (within past 12 months)
