Position Summary:
The RN provides care to patients requiring skilled nursing services, educates patients and caregivers, evaluates plan of care, and informing patient care team of any changes in patient status or plan of care. Skilled nursing services are provided in accordance with the plan of care and physician orders. RN is responsible for the development and revision of patients plan of care and obtaining updated orders until all skilled needs are met and patient is discharged. Specialized care may include but not limited to management of wound care, central lines, drains, feeding tubes, foley catheters, and ostomies.
Responsibilities:
Provide skilled nursing care as outlined in nursing care plan to include the following:
- Nursing services, treatment, education, and preventative procedures requiring specialized skill and ordered by the physician.
- Initiating preventative and rehabilitative nursing procedures as appropriate for the patient’s care and safety.
Qualifications:
- Valid RN IL license
- BLS
- Reliable transportation with current insurance coverage and driver’s license
- Experience with OASIS and Wellsky is a plus
- Experience working in home health or similar setting preferred.
