The Home Health RN is responsible for providing home health services and care to patients and their families. The care is provided at patient homes. RN is also responsible for identifying patient needs through continuum of care and quality assurance in coordinance with the DON and clinical team. The Home Health RN trains, directs, and provides clinical supervision for Home Health Aides and Home Makers, while ensuring patients quality of care, safety, and quality care outcomes.
