About the Job

ALL CARE and COMCARE home health provider group has an immediate opening for a PER DIEM (field-based) California-licensed Registered Nurse to work as a home visiting nurse for Riverside, CA and it's neighboring areas. Preferably looking for an IV nurse who is great at peripheral insertions and infusion via PICC/pump, competent with wound care, experienced in assessment/charting, a fast learner, and has good communication skills.

This opportunity is also open to a clinically experienced RN who is a self-starter in home health. Responsibilities:

• RN to undertake Admission, assessments, and supervisory visits/ follow-ups.
• Will coordinates and directs the home care patient's based on individual patient needs.
• Cares for a caseload of home health patients by evaluating the patient for appropriateness of home health and developing the home care plan in conjunction with the physician.
• Educates patients, families, caregivers and community providers to safely perform care.
• Assesses the home environment for safety, infection control, and community resource needs.
• Implement a plan of care.
• Case coordination.
• Attend team in-services.

Qualifications:

• Minimum of one (1) year relevant RN experience.
• Recent, relevant experience in a Medicare-certified home health agency as a case manager, or primary nurse is a plus
• Familiarity and work experience using home health software documentation application such as MyUnity or is willing to learn.
• Registered as a professional nurse to work in California.
• Valid California driver's license and current car insurance.
• CPR Certified.
• Current IV Therapy skills, with knowledge of venous access devices and ambulatory pumps.
• Knowledge regarding the impact of acute and chronic illness on the lives of patients and patient families in the community.
• Functions with a high degree of independence.
• Collaborates with other disciplines to provide care.
• Has excellent interpersonal and group process skills.

Salary: $85,000.00 - $150,000.00 per year
