Home Health Agency seeks a full-time R.N. for Luzerne/Lackawanna Counties. Salary is based on experience. Mileage reimbursement. Benefits for full-time status employees only.
Summary: The Registered Nurse plans, organizes, and directs home care services and is experienced in clinical nursing, with a strong emphasis on community health education/experience—Administers skilled nursing care to patients requiring intermittent professional nursing services. Teach and supervise the patient, family, and other nursing team members.
Responsibilities:
· Review the home health referral, updating and communicating any changes to other disciplines and clinical managers.
· Complete Start of Care promptly.
· Develop a rapport with the patient and family, ensuring privacy and confidentiality while obtaining accurate client history.
· Complete a physical assessment and communicate the findings on the plan of care.
· Conduct Home Health aide supervisory duties as required by state regulation.
· Document accurately and appropriately on the laptop during each visit.
· When appropriate, enter insurance authorization requests and continue to enter additional requests at the end of each authorization period.
· Organize daily assignments, setting priorities based on the individual and overall needs of the patient.
· Ensure the patients have prescribed medications and treatment supplies, updating files as changes occur.
· Identify appropriate actions that will provide continuity of care, an individualized teaching plan, and the involvement of the individual and family/caregiver whenever possible.
· Formulate a plan of care to achieve expected outcomes through short and long-term goals that are realistic and feasible in terms of resources, time, material, people, and cultural relevancy. Revise the plan of care in response to changes in the individual’s status or adjust to the individual’s response to care, and communicate the update with all disciplines, including clinical managers.
· Coordinate discharge planning with the direct involvement of client, family/caregiver
· Document any communication and orders from the physician
· Attend case conferences as scheduled, providing weekly client reports to the team.
· Maintain a high level of competency, growth, and development, including ensuring license and C.P.R. are current and in good standing at all times.
· Attend nursing staff meetings and educational in-services.
· Follows agency policies and procedures as per the Associated Family Home Care Policy and Procedure Manual
· Assists in the orientation of new employees on home visits and documentation
· Accepts On-Call responsibilities on a regular (rotating) basis
· Maintains awareness of community resources and utilize them appropriately to meet patient’s needs
Education:
Graduate of an N.L.N.-approved school of nursing
Requirements:
One (1) year of recent medical-surgical or Home Health Nursing experience or extensive educational training
Current license to practice as a Registered Nurse in the Commonwealth of PA
CPR Certification
Valid PA Driver’s License
Strong computer skills
