Job Title: Registered Nurse (RN) | Home Health Care & Hospice

Job Type: On-Call

Location: Somerset County | Somerville, NJ

Rates:

• Daily rate Based on Experience
• Additional Flat rate/per visit

Education/Experience:

• Current NJ RN License
• Valid Driver’s License required
• One year exp. in home health or hospice required

Job Summary: On call nurse triages after hour calls received from answering service. Communicates with patient, assesses situation, provides instruction and/or determines if visit is required.

Job Description:

• Provides safe on-call triaging for both Home Care and Hospice under medical direction and in accordance with recognized standards of nursing practice and established policies of the agency.
• Responds to call for assistance within 15-30 minutes.
• Assesses the needs of the client, develops plan of action, determines need for home visit, physician notification and/or 911, emergency room.
• If home visit required, assesses situation, determines plan of care, contacts physician as necessary, observes response and documents results.
