$4,000 Sign on Bonus. (Prorated for part time hires) Responsible for nursing assessment of clients in the home. Developing a plan of care (POC) with the physician, client, family, updating the POC, and changing the POC as needed by ongoing intermittent assessment and evaluation. Plans for coordinated nursing service for individuals and families. Appraises individual and family health needs and hazards (existing or potential). Provides intermittent nursing care to clients as well as teaching and supervision. Consults with and refers families to appropriate personnel within the Agency or other community resources.
