- Assists in the identification and outreach of medically complex patients.
- Collaborates with the multidisciplinary team in the development and implementation of the care plan.
- Engages patients by way of home visit, hospital visit and accompanying patients to Providers appointments as needed.
- Completes Vital Signs
- Identifies gaps in care and connects members with resources
- Facilitates telemedicine visits with members and providers
- Performs disease specific education
- Conducts safety evaluations in patients home
- Communicates all concerns to clinical team and manager as necessary, seeking support and guidance proactively.
- Provides reinforcement of an established treatment plan.
- Builds relationships within the community in order to identify resources that are available to the patients i.e.: DME vendors, Home Health Agencies, Area Aging Office etc.
Work is typically performed in an office environment. Accountable for satisfying all job specific obligations and complying with all organization policies and procedures. Hours: 8 AM to 4:30 PM (4-days per week) and 9 AM to 6:00 PM (1-day a week)
Travel: more than 50%
