    Our roots began in 1981 as the Montrose Clinic, with specialization in HIV education, testing, and treatment. Since then, the agency has expanded to >50 clinics in Houston, one in Baytown, two in Beaumont, and one in Deer Park with extensive services that include: Adult primary care, HIV/AIDS care, pediatrics, OB/GYN and maternity, dental, vision and behavioral health. We also service students within KIPP and YES Prep schools. Legacy is committed to driving healthy change in our communities.

    Job Description

    The Health Navigation Specialist builds strong relationships with community members and Legacy patients in order to help them access medical care, stay engaged in said care, and adhere to their medications for HIV prevention or treatment through Legacy Pharmacy partnerships. The Health Navigation Specialist works in clinic and community environments to seek individuals in need of prevention services and link them to Legacy for care. As an integrated member of the clinical care team, the Health Navigation Specialist assists patients with accessing and navigating the continuum of care within Legacy clinics. Current topics include (but are not limited to): HIV/STI testing, Pre-Exposure Prophylaxis (PrEP), Post-Exposure Prophylaxis (nPEP), HIV Treatment as Prevention (TasP), medication adherence, and general safer sex practices. *Position may be partially or fully funded by grant(s).

    Essential Functions

    · Implement health education programming in one-one-one and group settings to increase the knowledge of patients to prevent illness, manage health conditions, and improve overall wellness.

    · Use health education, navigation, disease self-management, goal setting, motivational interviewing, and other proven techniques to meet the specific needs and assist in setting goals related to one’s health.

    · Provide information, referrals, and assistance with linkage to services with the goal of improving patients’ independence and overall health; supporting their varying routes throughout Legacy.

    · Provide point-of-care testing for HIV and other health conditions in the community and clinic setting

    · Contribute to the development of new health education programming as identified from interactions with individual, clinic and community participants.

    · Serve as a member of an inter-disciplinary clinic and care team and promote effective working relationships with internal and external stakeholders.

    · Conduct brief assessments of patient needs (behavioral, social, and medical), readiness for health behavior change, and progress over time.

    · Establish and maintain referral processes, resource lists, and other community networks to effectively link patients to needed health information and services.

    · Work within the context of a primary care medical home, applying a team-based approach to care while working in partnership with patients/families to promote: timely access to care, understanding of patient care that is culturally sensitive and language appropriate, continuity of care, and the improvement of the whole-person through the teaching of appropriate self-management skills

    · Meet a high standard of customer care to ensure a positive patient experience.

    · Document all patient services in the electronic health record and other data systems as required.

    · Manage patient schedules and desktop in the electronic health record system.

    · Meet assigned performance metrics including those related to grant/contract requirements, if applicable.

    · Other duties as assigned.

    Education, Training and Work Requirements

    • High School Diploma or equivalent; and 3 - 5 years of relevant experience
    • Bachelor’s Degree in a related field with a minimum of 1 year of relevant experience preferred.
    • Basic Life Support (BLS) Certification
    • Ability to meet Legacy's Community Health's EPIC proficiency requirements.
