Community Health Worker

  • The Community Health Worker is responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors.

    Essential Duties and Responsibilities

    • Provide ongoing follow-up, basic motivation interviewing and goal setting with patients and families
    • Conduct intake interviews with patients, including enrolling and/or referring patients back into PCHS
    • Participate in insurance campaigns to include patients at transitions of life, such as aging into Medicare, aging off parent’s coverage, during Medicare open enrollment, patients eligible for low-income subsidies, etc.
    • Maintain knowledge of community resources appropriate to needs of patients and families
    • Follow up with patients via phone, home visits, and other settings patients can be found
    • Establish trusting relationships with patients and their families while providing general support and encouragement
    • Assist patients with completing applications and registration forms
    • Help patients connect with transportation resources and provide appointment reminders in special circumstances
    • Act as a patient advocate and liaison between the patient/family and community service agencies such as school, Department of Health and Human Services, Health Care for Homeless, hospitals, support groups, etc.
    • Familiar with using electronic health record (Ex. Athena and NextGen)
    • Other duties as assigned

    Supervisory Responsibilities

    This position has no direct reports.

    Qualifications

    • Successful completion of the CHW training program through Washington DOH within 120 days of hire (preferred)
    • Valid driver’s license (required)
    • Active CPR Certification within 30 days of hire (required)
    • Lived experience related to position (preferred)
    • If qualified, will become SHIBA and Medicaid certified.

    PCHS offers a wide variety of benefits through a customizable cafeteria plan.     For pay equity purposes, PCHS calculates salaries based on a wage scale that incorporates a standardized approach to compensation. Accordingly, PCHS does not engage in salary negotiations for non-provider staff.

    Range: $16.40 - $19.58 /hour DOE

Perform job with professionalism and good judgment.

    Work Environment and Physical Demands

    With multiple locations and settings, PCHS’ work environment and physical demands vary greatly depending on the employee’s role. Essential job functions include the following: The employee is required to talk, hear, see, smell, sit and stand. The employee is frequently required to move around the facility; reach with hands and arms; climb or balance and stoop, kneel, crouch, or crawl. The employee is frequently required to lift and/or move up to 10 pounds and occasionally required to lift and/or move up to 25 pounds.
