    The mission of Prestera Center is a united effort dedicated to helping people achieve their full potential and to be the preferred choice for behavioral health services.

    JOB SUMMARY

    The Community Health Worker will provide support through collaborating with partners and coordinating a variety of services for consumers and their families to have their needs identified and links them to medical, behavioral, and related social services and supports they need in order to promote a healthier, stable lifestyle.

    DUTIES AND RESPONSIBILITIES

    • Engages, collaborates, and communicates effectively with consumers and families, physicians, health care providers, and team members.
    • Schedule’s appointments and assures all parties have relevant consents and information needed.
    • Arranges transportation for consumers if needed.
    • Completes screenings as needed to identify needs of the consumer/family.
    • Contacts all referrals assigned within 72 hours and documents all activities per policy.
    • Provides comprehensive transitional care when the consumer receives services with another provider.
    • Links’ consumers and families to support groups, peer supports, and self-care programs as indicated by the treatment plan.
    • Participates in treatment plan meetings as indicated.
    • Provides outreach to consumers and referral sources.
    • Encourages consumer participation and compliance in all program services.
    • Educates and models healthy basic living and personal care tasks/behaviors for consumers.
    • Monitors consumers’ behavior and determines appropriate course of action with the treatment team when behavior needs are identified.
    • Attends all departmental and center meetings/trainings as assigned.
    • Writes professional, quality reports and correspondence.
    • Adheres to confidentiality, HIPAA, and risk management policies and procedures including but not limited to completing Incident Reports.
    • Utilizes technology provided to perform job efficiently and effectively.
    • Participates in other functional work by participating in related projects, as applicable and directed by Center leadership.

    SUPERVISORY RESPONSIBILITIES

    No direct supervisory responsibilities are required.

    Qualifications


    QUALIFICATIONS AND REQUIREMENTS

    EXPERIENCE, KNOWLEDGE, AND SKILL REQUIREMENTS:

    • Experience or prior training working with mental health and substance abuse population is preferred.
    • Knowledge of community resources preferred.
    • Ability to learn, navigate, and work daily in the EHR system effectively.
    • Must have a high level of interpersonal skills to handle sensitive and confidential situations.
    • Possess strong written and verbal communication skills in conjunction with strong time management and organizational skills with the ability to complete multiple tasks and high volume of work with deadlines.
    • Has shown ability to take initiative and respond quickly and accurately to partner and consumer inquiries.
    • Proficient in Microsoft Office applications, especially Microsoft Word.
    • Must be able to lift to 25 lbs.

    EDUCATION AND TRAINING REQUIREMENTS:

    • High School Diploma or General Educational Development Certificate required.
    • Associates degree preferred.
    • Maintains First Aid/CPR certification.
    • Maintains current crisis prevention/intervention training.
