JOB SUMMARY:
Tri Counties Community Action Partnership (TCCAP) is a 501c3 non-profit lead by community-minded professionals with a passion for building trauma informed, whole-person systems of care. As a community health worker (CHW) you will serve as a community liaison between the health patient, their care team, and their community. Your knowledge of current community programs and resources will be instrumental in developing well-targeted and effective programs.
CHW's are responsible for bridging conversations with patients and remove barriers that prevent individuals from accessing health and social services. CHW's meet their patients through in-person or online outreach and create a care plan with the patient to practice wellness activities that support their resiliency. The position may be stationed part-time at community providers offices, housing navigation centers, and/or home office. The CHW position is under supervision of the TCCAP Program Coordinator.
SCHEDULE:
Flexible schedule M-F between 8a-6p. Requires occasional rotating evenings and weekends 1-4x/month.
COMPENSATION:
$20- $25 per hour DOE- Bilingual Differential offered
Full time, 32 Hrs/Week. For employee benefits Full Time is considered 30 hours per week with paid holidays, medical, dental, vision, and paid time off.
NOTE:
Position will be based in Colusa County. Requires some telecommuting and access to broadband. Some travel required. Recruitment may be used to establish a list to fill future vacancies for the next six months.
