Community Support Team Substance Use Specialist

    SUMMARY
    The CST Substance Use (SU) Professional provides direct and indirect services to adult consumers with mental health, substance use, or co-morbid disorders who have complex and extensive treatment needs. This position assists the team leader with behavioral and substance use disorder treatment interventions. The CST SU Professional supports consumers challenged with MH, SU, or co-occurring issues in changing destructive behaviors, learning new coping skills, and in developing relapse prevention strategies. This position provides recovery-oriented interventions designed to sustain active commuity living on as independent a level as possible for each consumer. This position participates in intake, orientation, and treatment planning; screening and assessmnent; case management; service coordination, consultation, and referral; supportive counseling, education, and skills development; crisis management; and documentation. The CST Team SU Professional must be a CCS, LCAS, LCAS-A, or CSAC and meet the status of a Qualified Professional according to 10A NCAC 27G .0104. The CST Team must operate within the following staff-to-consumer ratios: 1-12 clinical staff to client ratio with a team maximum of 48 consumers.

    ESSENTIAL DUTIES AND RESPONSIBILITIES
    • Plan, coordinate and oversee initial and ongoing assessment activities.
    • Conduct or participate in ongoing clinical or substance use assessments considering the relationship between substance use and mental health. These include assessment of: psychiatric and substance use history (e.g., onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g., self-care, living situation, nutrition, money management); and family structure and relationships; ASAM level and stages of change
    • Develop client’s PCP using Person-Centered Thinking. Revise and update client’s PCP as necessary per result of ongoing assessment activities.
    • Organize and facilitate regularly scheduled treatment team meetings to facilitate person-centered planning, revisions and updates. Include formal and informal supports as appropriate.
    • Develop and maintain cooperative and collaborative working relationships with members of the client system.
    • Consult with community agencies and families to maintain coordination in the treatment process.
    • Serve as an FPS Ambassador by positively representing FPS in working with individuals, families and community partners.
    • Ensure interventions are based on recommendations from assessment activities, are supported by documented medical necessity, and are implemented as documented in the PCP
    • Provide individual supportive therapy (e.g. problem solving, role-playing, modeling and support), social skill development, and assertiveness training to increase client social and interpersonal activities in community settings.
    • Participate in treatment, rehabilitation (e.g. skill building, socialization, ADL’s, housing), and support services.
    • Provide ongoing measurement of client outcomes and evaluation of intervention effectiveness.
    • Make referrals to recommended services or community groups/supports as appropriate and ensure continuity of care as client moves across levels of care.
    • Develop and implement crisis stabilization plans for clients.
    • Perform “first responder” duties of crisis response as assigned.
    • Provide case management functions such as: linking, referring, arranging and coordinating services.
    • Identify and intervene to address barriers that impede the development of skills necessary for independent functioning in the community.
    • Provide information for court reports and other client related reports as required, and attend court as requested and/or needed.
    • Coordinate discharge planning for clients in hospital, residential services, or other levels of care.
    • Coordinate and assist clients with community re-entry following hospitalization, residential services, and other levels of care.
    • Learn and operationalize the program requirements, entrance criteria, continued stay criteria and discharge criteria for utilization of Community Support Team Services as outlined in Division (MH/DD/SAS) Clinical Coverage Policy.
    • Secure authorization of services according to established timelines and regulatory requirements as dictated by the Division (MH/DD/SAS), the LME/MCO, Value Options and/or other authorizing sources as required.
    • Maintain client confidentiality in adherence to HIPAA regulations.
    • Complete in a timely manner, accurate clinical documentation of all services, interventions and client related activities. Maintain this documentation in accordance with standards of FPS’ file management systems.
    • Cooperatively and actively participate in all assigned staff and supervisory meetings as well as in service trainings and staff development activities.
    • Work with supervisor to self-identify training and supervision needs, and ensure services to clients are provided only within the scope of current expertise and abilities
    • Consistently maintain established productivity requirements for delivering direct, billable services to assigned clients.
    • Perform shift management coordination with other CST team members according to established policies and procedures.
    • Provide on-call crisis intervention in coordination with other team members covering daytime, nighttime and weekend hours to ensure 24/7/365 coverage is available to consumers.
    • Participate in weekly staff organizational meetings and treatment planning review meetings as required.
    • Produce reports and submit reports as requested, and by the established due date.

    Must meet requirements for a CCS, LCAS, LCAS-A or CSAC in NC:
    • Certified clinical supervisor (CCS)" means an individual who is certified as such by the North Carolina Substance Abuse Professional Practice Board
    • "Licensed clinical addictions specialist (LCAS)" means an individual who is licensed as such by the North Carolina Substance Abuse Professional Practice Board.
    • “Licensed clinical addictions specialist associate (LCAS-A)” means an individual who is provisionally licensed as such by the North Carolina Substance Abuse Professional Practice Board.
    • "Certified substance abuse counselor (CSAC)" means an individual who is certified as such by the North Carolina Substance Abuse Professional Certification Board


    MUST MEET REQUIREMENTS FOR A QUALIFIED PROFESSIONAL IN NORTH CAROLINA PER 10A NCAC 27G .0104:
    Graduate of an accredited college or university with a Baccalaureate Degree in Social Work or Psychology, or a related human services field. Has 2 years of MH/DD/SA experience providing direct service to the population served.

    Graduate of an accredited college or university with a Master’s Degree in Social Work, Psychology, or a related human services field. Has 1 year of MH/DD/SA experience providing direct service to the population served OR has contracted with a professional licensure board.
    OR
    Graduate of an accredited college or university with a Baccalaureate Degree or a Master’s Degree in a field unrelated to human services. Has required number of years of MH/DD/SA experience providing direct service to the population served.

    PREFERRED EDUCATION AND EXPERIENCE
    • 2 years related experience and/or training, licensure preferred.
