    Job Details

    The Community Health Care Worker will work to screen all patients for Social Drivers of Health, including but not limited to food insecurity, housing, transportation, utility insecurity. The Community Health Care Worker will build up an external community based resource network and connect patients to the appropriate resources. The Community Health Care Worker will document results and make necessary internal referrals.

     

    Duties and Responsibilities:

    • Conduct community-based assessments and interview to identify and prioritize patient social services needs and objectives
    • Responsible for screening patients for social drivers of health including but not limited to food insecurity, housing, transportation and utility insecurity.
    • Build relationships with community based organizations that offer resources and services to address social drivers of health.
    • Connect patients identified with social driver of health needs with community based organizations.
    • Follow up with patients to make sure they were connected with the needed resources.
    • Work with the care coordination team to provide additional resources to patients as needed.
    • Track all assessments and outcomes in the designated electronic medical record.
    • Establish and maintain positive relationships with patients, visitors, and other employees
    • Contributes to and consistently applies Catholic Health policies, procedures, and benefits to all customers and/or employees without discrimination.
    • Employee conducts himself/herself in conformity with the HIPAA Compliance Program and applicable institutional policies and procedures for patient privacy.  In addition, employee conducts himself/herself in conformity with the Catholic Health Security policies and procedures.

    Position Requirements and Qualifications

    Education:

    • High School Diploma or equivalent
    • Community Health Worker Certificate

     

    Skills:

    • Quick learner and self-starter
    • Works well independently and within a team
    • Some community engagement experience, a plus
    • Familiarity with data entry and tracking, basic computer skills
    • Excellent verbal and written skills; presentation skills a plus
    • Experience working and/or engaging in priority neighborhoods
    • Strong interpersonal skills and the ability to build trust and maintain relationships with community partners, as well as patients.
    • Strong organizational skills with good follow-through to complete assignments within specified timeframes
    • Bilingual preferred
    • Demonstrate ability to work collaboratively
    • Demonstrate ability to work with families who face exceptional challenges

     

    Experience:

    One year of experience in community work preferred.



    Salary Range

    USD $25.00 - USD $28.00 /Hr.



    This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
