We are seeking a dedicated and compassionate Community Health Worker Liaison to join our team. As a Community Health Worker Liaison, you will play a vital role in improving the health and well-being of individuals and communities. Your main responsibility will be to provide support, education, and resources to individuals in need. Duties:
- Conduct assessments to identify community members' health needs and develop individualized plans to address those needs
- Conduct outreach activities to engage with community members and promote community health initiatives
- Assist community members in navigating the healthcare system and accessing necessary resources
-Provide case management and follow up to ensure that the linkages to services and supports addressed the participant's needs
- Build relationships with clients and provide ongoing support and guidance
- Assist clients in accessing healthcare services, including scheduling appointments and navigating insurance coverage
Requirements:
- Previous experience in healthcare or related field
- Project coordination experience and organizational skills
- Ability to manage multiples projects with attention to detail and meet deadlines
- Strong communication skills, both written and verbal
- Ability to work independently and as part of a team
- Excellent organizational skills with attention to detail
- Passion for helping others and promoting community health
- Bachelor's degree in related field preferred or commensurate experience
-Previous experience in the human services and social work preferred
