Position Status: Part-Time (10 hrs / week)
FLSA Status: Non-Exempt
Compensation: $17.79-$19.23 per hour, commensurate with qualifications; access to opt-in to retirement plan and free Employee Assistance Program access, as well as paid sick time.
Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI serves the most vulnerable individuals, families, and communities with critical services and programs. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – the poor and working poor, low-income people with chronic illnesses, persons with criminal justice involvement, substance users, domestic violence survivors, people of color, persons living with HIV/AIDS, and the LGBTQ community.
POSITION SUMMARY: The Engagement and Support Services (ESS) Program assists People Living with HIV to improve overall health and establish self-management through case management, peer navigation, and health education services with a focus on improving medical outcomes, achieving viral suppression, and enhancing self-management skills. The Community Health Worker (CHW) position was created using the research and belief that involving people who have their own lived experience with substance use, mental health conditions, and/ or HIV is invaluable in reaching and helping the population. The CHW mentors and guides individuals using their experiences to support people in a way that shows people that living a healthy life with HIV is not only possible but sustainable. The CHW maintains appropriate positive, professional interpersonal relationships with staff, participants, volunteers, and the community. The CHW will be well versed in all CHASI services to refer and link clients appropriate to services. The CHW’s responsibilities will align all activities with CHASI’s mission, core values, and culture.
